Fairview Seminary Home

Long-term skilled nursing care and short-term transitional care

Introduction
If you or a loved one can no longer live independently, Fairview Seminary Home offers long-term skilled nursing care. We also have transitional care for patients recovering from a serious illness, injury or surgery. When you need extra time to restore your skills in a supervised setting after being released from the hospital, think of transitional care as the bridge that gives you time to re-learn the skills you need to return home and live on your own.

Treatments we offer

  • Rehabilitation after joint replacement surgery
    With an on-site rehabilitation gym and daily visits from physical therapists, Seminary Home provides the ideal environment for patients recovering from joint replacement surgery.
  • Physical therapy is offered seven days a week.
  • Speech therapy
  • Occupational therapy
    We have a model home to help patients recovering from serious illness who need to relearn or practice the skills they need to return home and live on their own.
  • Long-term skilled nursing care for patients unable to live independently
    The 85-bed long-term care facility provides a caring environment for seniors who find it difficult to live a totally independent lifestyle
